The piles extended to the ceiling and employees were required to walk in between the piles to access work areas.  Some of the tires had collapsed and fallen on the sides of the piles.

Warehouse:  Toyota forktruck, serial number 60489, was found to be emitting an excessive amount of carbon monoxide.

The conveyor system was not provided with a pre-start up alarm to alert employees working in the area that they system was about to be started.  B.  The emergency stop devices located on the Feed and Loop sections of the conveyor were not operable.  C.  The start end of the Steel Conveyor was not provided with a guard and/or emergency stop device.
