29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On or about 07/11/2014, at the warehouse, employees were operating forklifts and had not been properly trained as required by the standard.  Lockwood Flooring was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 315896241, citation number 1, item number 2a and was affirmed as a final order on 10/03/2011 , with respect to a workplace located at 8249 Brentwood Industrial Blvd., St Louis, MO.
