29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:          a.) Homerwood Hardwood Flooring, Inc. 1026 Industrial Drive, Titusville, PA, 16354, Warehouse, on or about July 14th, 2014, and times prior thereto:  Company employees utilized liquid propane powered Hyster forklifts throughout the warehouse area of the facility.  Hyster forklift (Model S60 XL; Serial# C187V141865) was exhausting large amounts of Carbon Monoxide and needed mechanical repair.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:     a.)    Homerwood Hardwood Flooring, Inc. 1026 Industrial Drive, Titusville, PA, 16354, Warehouse, on or about July 14th, 2014, and times prior thereto:  Company employees were exposed to Carbon Monoxide at various levels from propane operated forklift exhaust.  The company had not conducted air monitoring or any other method of monitoring to determine the employees exposure levels to Carbon Monoxide.

Homerwood Hardwood Flooring, Inc. 1026 Industrial Drive, Titusville, PA, 16354, Production areas, on or about July 14th, 2014, and times prior thereto:  Company employees were working with and around hazardous substances including, but not limited to Bayhydur 302 (Aliphatic Polyisocyanate) and liquid propane.  A written Hazard Communication Program had not been developed.
