29 CFR 1910.1025(d)(6)(ii): Where the initial determination or subsequent monitoring revealed employee exposure to lead to be at or above the action level but below the permissible exposure limit; monitoring was not repeated at least every six months:  On or about July 16, 2014, an employee torch cutting materials in the scrap yard was exposed to lead at an 8-hour time weighted average of 0.0411 mg/m3 for the 448 minute sampling period, which is 1.37 times  the action level of 0.03 mg/m3. Zero exposure is assumed for the 32 minutes not sampled.  Exposure monitoring has not been conducted every 6 months.

29 CFR 1910.1025(j)(2)(i)(A): The employer did not make available biological monitoring in the form of blood sampling and analysis for lead and zinc protoporphyrin levels at least every 6 months to each affected employee:   On or about July 16, 2014, employees that may be exposed to lead above the action level for 30 or more days per year were not offered blood lead and ZPP sampling and analysis every 6 months.
