In the Olefins-1 Unit at Formosa Plastics Corporation located in Point Comfort, Texas, the employer operated process piping with deficient piping supports; such as but not limited to the FG331-1" Fuel Gas vent piping system.    b.  In the Ethylene Glycol Unit at Formosa Plastics Corporation located in Point Comfort, Texas, the employer operated process equipment with broken sight glass; such as but not limited to the G-V-223 Regenerator Reboiler Condensate Tank.  c.  In the Olefins-1 Unit at Formosa Plastics Corporation located in Point Comfort, Texas, the employer operated deficient process equipment such as but not limited to the Dryer Regen Heater 3122-C and control valve LV-31203.
