29 CFR 1926.752(e): Where the employer elected to use alternate means and methods to provide employee protection in accordance with 1926.753(c)(5),1926.757(a)(4) or 1926.757(e)(4) ; a site specific erection plan was not developed by a qualified person nor was it available at the worksite:  On the jobsite 15705 Harlan Lewis Road, Bellevue, NE; Digester #4 of the Papio Creek Water Treatment Facility: The employer is failing to ensure that protective measures used to protect employees were developed by and provided to employees by a qualified person. The employer was placing metal sheets (decking) with a 15 ton National Series Crane, model # 562C, serial# 31393. Employees were instructed to use crane to lift up on the sheets (decking) after they were placed in order to remove bowing between structural members of the float. Once the sag was taken out, employees would place a cross beam across the span and weld the center of the sheet to the cross beam to hold in place. On or about July 15, 2014 an employee of the company was struck in the head when the lifting eye weld broke while attempting to connect a sheet to a cross beam.
