29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:  On or about July 24, 2014, in the blending area, the employer failed to ensure that hook and hoist chain inspections were performed frequently. Employees used the hoist 8 or 9 times a day to lift totes that weighed between 1,500 - 2,000 pounds.

29 CFR 1910.305(a)(2)(i)(B): Temporary electrical power and lighting installations of 600 volts, nominal, or less was used for a period exceeding 90 days.    On or about July 24, 2014, in the production line area, the employer failed to ensure that permanent electrical outlets were installed. Extension cords were being used a permanent wiring.  1) on line 4 it dropped down to the carton former machine. 2) on line 6 an extension cord was attached with zip ties and ran from the outlet along the machine and was being used as a power source for a 115 volt vibrator.
