29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. On or about July 23, 2014, and times prior, employees were exposed to injuries in that the employer had not established an energy control program which consists of energy control procedures, employee training and periodic inspections for equipment at the facility including the coil line machine. Employee(s) were performing maintenance on this machine without utilization of lockout/tagout.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): a. On or about July 23, 2014, and times prior, employees were exposed to electrical injuries while working near an open electrical panel containing fuses and other components on the coil line machine where a lock and tag were not placed on the disconnect of the coil line unit, prior to removal of panel cover and worked near the open panel. The employer also did not verify that the circuit was deenergized prior to removing the panel and working near it.

On or about July 23, 2014, and times prior, employees were exposed to injuries from flying debris and particles while the operating Craftsman Drill Press that did not have guards around the chuck and bit. b. On or about July 23, 2014, and at times prior, employees were exposed to injuries from nip points while operating the Lock Former Machine (Pittsburg machine) that did not have guards around the rotating parts. c. On or about July 23, 2014, and at times prior, employees were exposed to injuries from exposure to in-running nip points while operating the three roll bending machine that did not have guards around the rollers. d. On or about July 23, 2014, and at times prior, employees were exposed to injuries while operating the ROTO DIE press brake that did not have guards around the point of operation. e. On or about July 23, 2014, and at times prior, employees were exposed to injuries from exposure to in-running nip points while operating the combination Machine (crimping machine) that did not have guards around the point of operation where the sheet metal is crimped. f. On or about July 23, 2014, and at times prior, employees were exposed to injuries from nip points while operating the coil line machine that did not have guards around the rollers.
