29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about July 31, 2014, at the job site, on the 2nd floor, the employer did not ensure workers were protected from falling down the elevator shaft by installing and maintaining guardrail systems. Workers were walking /working around and about an unguarded opening of an elevator shaft while performing construction activities including but not limited to hanging drywall, rough-in plumbing, electrical, and sprinkler system. This condition exposed employees to the hazard of falling greater than 14 feet through the elevator shaft opening which was greater than 5 feet wide to the elevator shaft pit below.
