29 CFR 1910.22(b)(1): 29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:     On or about August 7, 2014, at the ORO NEGRO UNIT 3, well number 10H, the walk way around the shale shaker area on the platform had protruding metal pipe sticking out as high as six inches at various points in the walk way. Employees exposed to a tripping hazard.

29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, shall be guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings:     On or about August 7, 2014, at the ORO NEGRO UNIT 3, well number 10H, the employer failed to ensure the pit behind the tanks was guarded. Employee(s) exposed to a fall hazard. The pit was approximately nine feet deep.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:     On or about August 7, 2014, at the ORO NEGRO UNIT 3, well number 10H, the platform on Rig #5, had a  floor hole that was not covered. Employees exposed to a tripping hazard. The floor hole was measured at 6.5 inches by 10 inches.

29 CFR 1910.23(b)(1)(i): Wall opening(s) from which there was a drop of more than four (4) feet, were not guarded by a rail, roller picket fence, half door, or equivalent barrier:      On or about August 7, 2014, at the ORO NEGRO UNIT 3, well number 10H, the platform on Rig #5, there was no adequate railing on the corner of the platform above the tanks. There was a gap between the platform and the chain.  Employees exposed to a fall hazard of eight feet.
