29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   On or about August 13, 2014 employees were exposed to struck-by hazards while working along the bottom of an excavation installing a concrete manhole box without having a safe means of egress along the following sections of the excavation:      a)    Along the west end section next to the concrete manhole box, the excavation area measured approximately 9 deep X 12 wide.     b)    Along the east end section next to the installed PVC, the excavation area measured approximately 6 deep X 5 wide.     The overall length of the excavation measured approximately 40' long.
