29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about August 13, 2014 employees were exposed to struck-by hazards while working along the bottom of an excavation installing a concrete manhole box without having a safe means of egress along the following sections of the excavation: a) Along the west end section next to the concrete manhole box, the excavation area measured approximately 9 deep X 12 wide. b) Along the east end section next to the installed PVC, the excavation area measured approximately 6 deep X 5 wide. The overall length of the excavation measured approximately 40' long.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 13, 2014 employees were exposed to struck-by hazards while working along the bottom of an excavation installing a concrete manhole box without having any type of protective systems along the following sections: a) Along the west section of the excavation, employees were working in and around the concrete manhole and the unprotected wall of the excavation. b) Along the east section of the excavation, employees were working in-between the PVC conduit system and the unprotected wall of the excavation.
