29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:   (a) On August 12, 2014 the inspection revealed that employees were potentially exposed to serious injury from the use of a chain sling to secure an attachment for lifting jersey barriers that was rigged by inserting the hook back into one of the chain links.
