OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard:       On or about August 18, 2014, and at times prior thereto, employees were exposed to a fall hazard while moving proppant bags without fall protection.    (a) Proppant bag storage warehouse, West side of the warehouse, employees standing on forklift tines were exposed to a fall hazard while being lifted to climb on top of proppant bags without fall protection.      (b) Proppant bag storage warehouse, West side of the warehouse, employees moving proppant bags were exposed to a fall hazard greater than 4 feet while standing on proppant bags without fall protection.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:   Proppant bag storage warehouse, West side of the warehouse, on or about August 18, 2014, and at times prior thereto, employees moving proppant bags were exposed to a struck-by hazard from proppant bags not being properly interlocked to prevent them from falling.
