29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  A.)  Chester Miller dba Chester Miller Construction, 357-359 West 7th Street, Erie, PA  16502, on or about August 18, 2014 and times prior thereto:  Two workers were performing roofing activities from a ladder jack scaffold at a height of 20 feet.  The workers were not utilizing a means or method of fall protection, including, but not limited to, personal fall arrest system.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A.)  Chester Miller dba Chester Miller Construction, 357-359 West 7th Street, Erie, PA  16502, on or about August 18, 2014 and times prior thereto:  A worker was exposed to a 20 foot fall hazard from the roof of a residence while performing roofing activities.  The worker was not utilizing a means or method of fall protection, including, but not limited to, personal fall arrest system, guardrails, and/or safety net system.
