29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) Concrete Entrance Road - On or about August 19, 2014 employees were exposed to impalement hazards while working near reinforcing steel and they were not protected by caps or other guards over the ends of the rebar. (b) Lay Yard of Welded Rebar Assemblies - On or about August 19, 2014 employees were exposed to impalement hazards while working near reinforcing steel while working with or retrieving materials from the lay yard. Employees were not protected by caps or other guards over the ends of the rebar. (c) West Side of Western Building Concrete Pad - On or about August 19, 2014 employees were exposed to impalement hazards while working with concrete forms near protruding reinforcing steel. Employees were not protected by caps or guards over the ends of the rebar.

(a) Job Trailer - On or about August 19, 2014 employees at the site were handling hazardous chemicals, such as, diesel fuel, oxygen and acetylene gases. Employees did not have access to a list of the hazardous chemicals on-site referenced to a safety data sheet.
