Chippewa Concrete Services, was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1910.134(d)(1)(iii), which was contained in OSHA Inspection Number 315466201, Citation Number 1, Item Number 1, Issued on 7/13/2011, and was affirmed final order on 8/1/2011, with respect to a workplace located at Entrance Ramp for I94, Oakdale, WI 54649.
