29 CFR 1926.602(c)(1)(ii): No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval:     a) Near the west side of Lilac Road, a bolt about 4.25 inches long by 3/8 inch diameter with two hex nuts on one end was used in lieu of the manufacture's retaining pins or equivalent to secure a bucket on a Caterpillar excavator, model number 303.5ECR, serial number RKY01455, exposing employees to the hazard of being struck by the bucket in the event the bucket separated from the excavator boom.     b) Near the west side of Lilac Road, a nail was used in lieu of the manufacturer's retaining pins or equivalent to secure the pin in a ripper to a Caterpillar backhoe, model number 303.5ECR, serial number RKY01455. Employees were exposed to the hazard of being struck by the ripper in the event the ripper separated from the backhoe.
