29 CFR 1910.179(g)(2)(i): Electrical equipment was not so located or enclosed that live parts would not be exposed to accidental contact under normal operating conditions:  On or about June 11, 2014, and at times prior, for employees working in close proximity to live crane rails where an employee brushed up against one and received an electric shock of 480 volts while performing work for Action Door at Epic Steel located at 2041 West 17th Street in Cleveland, Ohio.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    On or about September 5, 2014, and at times prior, for the employees exposed to potential struck-by hazards where the work rest was approximately one quarter inch (1/4") away from the wheel of the bench-top grinder in the shop of Action Door located at 201 East Granger Road in Brooklyn Heights, Ohio.
