29 CFR 1910.23(a)(5): Every pit and floor opening was not adequately guarded by standard railing or any other effective means. Employees were exposed to a 15-18 foot fall hazard when required to stand beside an unguarded basement opening to perform cleaning operations of the Baler. During cleaning operations, employees were responsible for guiding/directing the crane operator to collect material from the basement. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were exposed to a fall hazard when using the north stairway of the Gallan Henning Baler. The stairway has eight treads in total, with two of the treads vertically stacked at a different angle from the remaining six treads.
