29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:     The employer did not ensure every work platform was effectively guarded.  Employees were required to stand on the Old Shear box to perform servicing operations, which exposed employees to a 10 foot fall hazard. 29 CFR 1910.24(e): The fixed stairs were not installed at angles to the horizontal between 30 and 50 degrees as outlined, but not limited to the combinations given in Table D-1.    Employees were exposed to a fall hazard when using the north stairway of the Gallan Henning Baler. The stairway has eight treads in total, with two of the treads vertically stacked at a different angle from the remaining six treads.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
