29 CFR 1918.86(c): When the design of a ramp prevented physical separation, positive means were not established to prevent simultaneous uses of the ramp by vehicles and pedestrians.      (a) On October 7, 2014 the inspection revealed that employees were potentially exposed to serious injury due to an access ramp that was shared by both pedestrians and vehicles without physical separation or a positive means to prevent simultaneous use.

(a) On October 7, 2014 the inspection revealed that the written emergency action plan was not kept at the workplace. Located at berth 601 of terminal 6 in Portland, Oregon during ro-ro operations on the Kaijin Panama.
