29 CFR 1926.651(c)(1)(i): Structural ramps used for access or egress of equipment were not constructed in accordance with the design:    a) Diamond and Spruce Streets, Pottstown, PA, Worksite - on or about October 8, 2014, employee were accessing and egressing an excavation via the remnants of a previously designed ramp built into the former protective system for the excavation.  This would hamper employee exit from excavation in the event of an emergency.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:    a) Diamond and Spruce Streets, Pottstown, PA, Worksite - on or about October 8, 2014, employees were working in an excavation where sections of loose asphalt debris placed on the excavation face posed a hazard of rolling into the excavation.
