29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. Barge 50 shop: On or about October 16, 2014 and times prior thereto, an arc welding cable had a cut to the conductor two feet from the electrode holder. This exposed employees to potential shock from coming in contact with the conductor during welding operations.

29 CFR 1915.115(a)(1): When used to transfer materials or equipment from or to a vessel, cranes which are placed onboard barges, other vessels, or on wing walls of floating dry docks, were not tested and certified in accordance with the standards provided in 29 CFR 1919 of Hoisting and Hauling Equipment by accredited persons. Barge 50: On or about October 9, 2014 and times prior thereto, the employer did not have his cranes inspected by a certifier that was certified to inspect cranes under the floating crane and derricks. This exposed employees to potential struck-by hazards from a potential tip over or dropped load from working outside the parameters of lifting a load on a barge.
