He was not wearing eye protection.

The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:    On or about 10/15/2014, Park Ave.    a) The employee was using a Rigid 7 14" skilsaw to cut plywood. He was not using a ground fault circuit interrupter.

The roof measured 10' 3" high, 9' wide and 5' long. He was not protected from falling to the ground below.
