29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a.  On or about October 16, 2014, at the shredder area of the above addressed jobsite, employees were exposed to a fall hazard while working in a platform that lacked guardrails on one of its sides.

On or about October 16, 2014, at the tool room of the above addressed worksite, employees were exposed to an electrical hazard while working in close proximity of an electrical panel board which had unused openings not covered.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a.  On or about October 16, 2014, at the above addressed worksite, oxygen cylinders were stored in close proximity of diesel fuel and waste oil, exposing employees working in the area to a fire hazard.
