29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.    a) Production department - Employer failed to keep the walkway of the catwalk clear and free of obstruction.   Employees were exposed to trip hazards as flexible cords that supplied power to a lamp and hoist were obstructing the walkway.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   a) Production department  An employee working within the mix tank area near the Cleve-Vibro 9-14 vibrator was exposed to continuous noise at 100.3% of the permissible daily exposure of 85 dBA(8-hour time weighted average) or an equivalent sound level of 90.03 dBA during the 363 minute sampling period on November 4, 2014. The exposure calculation included a zero increment for the 117 minutes not sampled. The employee had not received an annual audiogram.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:    a) Production department - The employer failed to provide a danger sign for a 30 ton mixing tank that was deemed a permit confined space. Employees were working near the tank while mixing liquid aluminum.
