29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.   The employer does not ensure that covers and/or guardrails are provided to protect personnel from the hazards of open pits.  This violation was observed on or about October 17, 2014, on the grease processing side where employees were exposed to fall hazards when performing routine job duties of discharging grease into a pit without guardrails on the east, south and west sides of the grease pit.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that covers and/or guardrails are provided to protect personnel from the hazards of open pits.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails.       The employer does not ensure that every flight of stairs having four or more risers is equipped with a standard stair railing or standard handrail on all open sides as specified in paragraphs (d)(1)(i) through (v) of this section.  This violation was observed on or about October 17, 2014, near oil pit #3 where employees were exposed to fall hazards when accessing the pit for waste water samples without standard handrails on both sides of the stairs.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that every flight of stairs having four or more risers is equipped with a standard stair railing or standard handrail on all open sides as specified in paragraphs (d)(1)(i) through (v) of this section.
