29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees operating metal machining equipment were exposed to amputation hazards and flying chips and the employer did not ensure that adequate guards were used: a. The Bridgeport vertical mill was not guarded from flying chips during metal machining activities. b. The spindle on the Dean Smith & Grace type 13-1 lathe was not guarded. c. The spindle on the Dean Smith & Grace type 21 lathe was not guarded. d. The spindle on the 17 inch Turn-nado lathe was not guarded. e. The unused portion of the HEM horizontal band saw was not able to be guarded because the guard was missing. f. The unused portion of the Dayton 10 inch miter band saw was not guarded because the guard was missing.
