29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:  On or about October 28, 2014, and at times prior thereto, at 106 Sunland Park Ave., an employee was observed working off a JLG T350 Tow-Pro Boom Lift without any form of fall protection and was exposed to a fall hazard of greater than six-feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.  On or about October 28, 2014, and at times prior thereto, at 106 Sunland Park Ave., an employee was observed working off a JLG T350 Tow-Pro Boom Lift without any form of fall protection and was exposed to a fall hazard of greater than six-feet.
