29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:  (a)  Location - Tool Room On or about 10/31/14, the shared-work bench was covered with various gas service parts including, but not limited to, large pipe flanges, pipes and other equipment.

29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:  (a)  Location - Somerville, MA - NSTAR Gas Site On or about 10/31/14, in the following instances, the employer did not properly record the any calendar days away from work on the 2014 OSHA 300 log:          1)  Case # 03355, on 9/11/14, an employee received an amputation when a steel pipe flange spacer fell on the tip of his middle finger on his right hand resulting in 116 days away from work and no restricted days.  The employer had not recorded any days away from work.  The employer entered a check mark in column (I) and 50 days for Job Transfer or Restriction in column (L).       Abatement Certification and Documentation is required.
