29 CFR 1910.23(b)(1):  Every wall opening from which there is a drop of more than 4 feet was not guarded:    West wall of the mud house:  On or about October 31, 2014, and times prior to, employees operating the rig's drilling mud system during the drilling of an oil/gas well worked in the mud house dispensing drilling mud materials.  The mud house west wall door was open without a guard rail system installed exposing the employees to the hazard of falling over 4 feet.

This exposed the employees to the hazard of falling over 4 feet.

The employees were provided with an emergency escape system (Geronimo) that had an inadequate ground anchorage.  This exposed the employees to fall and struck by hazards

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use      The mud pits caustic dispersal area: On or about October 31, 2014, and times prior to, employees operating the rig's mud system during the drilling of an oil/gas well manually added drilling mud materials to the drilling mud.   No shower or eyewash was installed in the area exposing the employees to hazards from prolong contact with the corrosive material including but not limited to caustic soda and hydrated lime.
