29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) Jobsite, 1901 Lombard St., Philadelphia - The work areas at the front of the building including the areas around building entrances were not kept free of bricks and debris, exposing employees to trip hazards.  Observed on or about 10/17/14.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:    a) Jobsite, 1901 Lombard St., Philadelphia - The holes in the ground around the basement doors along the front of the building were not covered.  Employees, including laborers and employees of masonry and other subcontractors who were working and/or walking through the area in front of the residences on Lombard Street, were not protected from falling into the holes, exposing employees to a fall hazard of 6 feet.  Observed on or about 10/17/14.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) Jobsite, 1901 Lombard St., Philadelphia - The entrances to the front of the building were twenty four (24) inches above ground level.  The employer did not provide ramps, runways or sloped embankments, for employees access/egress to and from the building. Observed on or about 10/17/14.
