29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:    On November 4, 2014, North and West edge of a low-sloped flat roof, the employer did not ensure a warning line was completely erected around all sides of the roof area where employees were installing a new roof system.  This condition exposed employees to the hazards of falls from an elevation of 24 feet from roofline to ground below.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:     On or about November 4, 2014, at the edge of the low-sloop roof, the employer did not ensure that the safety monitor was not distracted from his monitoring activities. The Safety Monitor was kneeling down and at times bent over at the leading edge of the roof helping an employee while other employees to his North and East were walking and working on the roof greater than 15 feet behind him. This created a condition which exposed employees to unprotected falls of 24 feet to ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      The CHICO'S ROOFING CONSTRUCTION LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 314637232, citation number 01, item number 003 and was affirmed as a final order on 12/16/2010, with respect to a workplace located at 160 Joe Frank Harris Parkway, Cartersville, Georgia 30120-2854.    On or about November 4, 2014, on a low-sloped, flat roof, the employer did not ensure employees were protected from falling by use of fall protection systems, while they were installing Thermoplastic polyolefin (TPO) material to the roof. Employees were observed roofing in areas of the roof e.g. north and west sides where there was no guardrail, safety net or warning line system in place or in use. This practice exposed the employees to fall hazards of 24 feet to the ground below.
