29 CFR 1926.1428(a): The employer of the signal person must ensure that each signal person meets the Qualification Requirements (paragraph (c) of this section) prior to giving any signals. This requirement must be met by using either Option (1) or Option (2) of this section.    a.  On or about November 7, 2014, located at 43701 Industrial Park in Cadiz, Ohio, employees were installing insulation on flanges on T501 demethanizer tower in the cryogenic area of the site.  The employees were performing this work from a man basket that was attached to a Manitowoc crawler crane, model number 999, serial number 9991087.  The employees in the man basket were providing radio and hand/arm signals to the crane operator without being qualified by a third party qualified evaluator or a competent qualified evaluator selected by the employer, thereby exposing the employees to a fall hazard up to 125 feet.
