29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a) Facility Wide - Winzeler, Inc. dba Winzeler Gear, Inc. failed to conduct a periodic inspection of the energy control procedures at least annually.   In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   a) Production Area - On November 13, 2014, Winzeler, Inc. dba Winzeler Gear, Inc. did not ensure sufficient access and working space was provided at an electrical panel near the Engel plastic molding machine (#5).  The electrical panel was blocked by boxes being stored in front of the panel.    b) Production Area - On November 13, 2014, Winzeler, Inc. dba Winzeler Gear, Inc. did not ensure sufficient access and working space was provided at an electrical panel near the Engel plastic molding machine (#10).  The electrical panel was blocked by boxes being stored in front of the panel.
