29 CFR 1926.405(d): Panelboards mounted in cabinets, cutout boxes or enclosures designed for the purpose were not dead front:   At this commercial renovation site, in Building A, Section 2, a circuit breaker box had exposed wiring and was not protected by a dead front cover. Employees were exposed to electrical shock and burns.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  At this commercial renovation construction site, the flexible cord attached to a Dewalt circular saw had its outer insulation pulled away from its plug so that strain relief was not provided.
