29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) The employer had not developed a written energy control program for servicing and maintaining machines and equipment in the facility, such as the KHS Bartelet IM 6-17 Form, and Fill and Seal machine and the Barry-Wehmiller Ultramax CM bagging machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) One of the KHS Bartelet IM6-17 Form, Fill and Seal machines, s/n 3625, was disassembled and undergoing maintenance or repair. The machine was isolated from the main electrical source (disconnect turned off) but was not locked or tagged out to prevent unauthorized re-energization.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about November 21, 2014, the inclined conveyor, the lower guide roller, equipped with a sprocket to engage the plastic conveyor belt was not guarded exposing workers to in-running nip point hazards during operation and sanitation activities.    (b) On or about November 21, 2014,  Barry-Wehmiller Ultramax CM bagging machine, the polycarbonate door interlock switch was inoperative allowing workers access into the area exposing employees to in-running nip point and caught-in hazards.
