29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:  (a) Mud Tank Area: The employer requires an employee to use respirators such as a North, 5400 series, full face air purifying respirators with North 7583P100L combination cartridges while adding Caustic Soda, which contains Sodium Hydroxide.  The respirator was covered inside and outside with dried materials and not maintained in a sanitary condition, as determined on or about 11/30/2014.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge of how to inspect, put on, remove, use, and check the seals of the respirator:  (a) Mud Tank Area: The employer requires an employee to use respirators such as a North, 5400 series, full face air purifying respirators with North 7583P100L combination cartridges while adding Caustic Soda, which contains Sodium Hydroxide.  The employees interviewed could not demonstrate knowledge on how to inspect and maintain their respirator, as determined on or about 11/30/2014.  ABATEMENT CERTIFICATION REQUIRED
