29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:          a) Street side of building - Employees were exposed to the hazard of falling from an elevation of approximately 16 feet while working from pump-jack scaffolding without the benefit of fall protection at the end of the scaffold.    b) Yard side of building - Employees were exposed to the hazard of falling from an elevation of approximately 16 feet while working from a scaffolding platform without the benefit of fall protection.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):           a) Employees were exposed to the hazard of falling approximately 22 feet where fall protection was not provided during the performance of carpentry work from the roof of the building.           b) Employees were exposed to the hazard of falling where fall protection was not provided at and around window and wall openings on the second story of the building.           c) Employees were exposed to the hazard of falling approximately 9 feet while performing carpentry work from the attic space/deck of the building without the benefit of fall protection at open sided floors.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
