On December 3, 2014, at the main entrance, two stacks of wooden empty pallets placed on the top of the intermodal container were loose and not secured, exposing employees to the hazard of being struck by pallets.

29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1):  a. On December 3, 2014, the employer did not provide forklift training and evaluation for the forklift operators at the Tafuna warehouse. Employees operate forklift everyday.
