29 CFR 1910.178(l)(1)(i):    The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   (a) On or about December 1, 2014 at Best Service Stores at 8250 NE Underground Rd Pillar 156G, Kansas City, Missouri.-employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operators had not completed a competent training program.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

(a) On or about December 1, 2014 at Best Service Stores at 8250 NE Underground Rd Pillar 156G, Kansas City, Missouri.-employees were exposed to a struck by hazard when operators did not determine if operating controls to include steering, braking, lift controls were functioning as intended.   daily inspection of a Hyster H80XM fork truck was not accomplished.    Abatement certification is required for this item  use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose

29 CFR 1910.305(g)(1)(iv)(A):    Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   (a) On or about December 1, 2014 at Best Service Stores at 8250 NE Underground Rd Pillar 156G, Kansas City, Missouri.- employees were exposed to an electrical hazard when an extension cord was  used as a substitute for fixed wiring for a microwave and a refrigerator.     Abatement certification is not required for this item-Corrected during inspection.
