29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about December 5, 2014 during the installation of roofing materials at a Rainforest Car Wash the employer did not ensure that employees were protected from a fall hazard of up to approximately 13.5 feet by utilizing a fall protection system/device.
