29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Worksite, Burger King Restaurant, 700 Rt. 109., Lindenhurst, NY- Employee replacing light bulb and ballast for a parking lot light pole was observed operating an aerial lift approximately 15' above the ground below without wearing a lanyard or harness; on or about 12/4/14.     Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
