29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  In the machine shop a quad plex knockout receptacle outlet box lying on the floor was used to service a Dewalt DW 758 heavy duty grinder, a Westco Machinery vertical sander and a Black & Decker Professional chop saw (SN 167118).  The knockout receptacle outlet box was not used according to the manufacturer's instructions.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  In the machine shop the plug servicing the Black & Decker Professional chop saw (SN 167118) was missing a ground pin.
