29 CFR 1904.10(a):  Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log:  a) At the facility, where the employer did not record a standard threshold shift (STS) on the 2014 OSHA 300 log.  A STS was not recorded on December 29, 2014, where an employee had a STS in the left ear.  b) At the facility, where the employer did not record a STS on the 2015 OSHA 300 log.  A STS was not recorded on January 14, 2015, where an employee had a STS in the left ear.  c) At the facility, where the employer did not record a STS on the 2015 OSHA 300 log.  A STS was not recorded on January 8, 2015, where an employee had bilateral STS in his ears.  d) At the facility, where the employer did not record a STS on the 2015 OSHA 300 log.  A STS was not recorded on January 30, 2015, where an employee had a STS in the right ear.  e) At the facility, where the employer did not record a STS on the 2015 OSHA 300 log.  A STS was not recorded on January 27, 2015, where an employee had a STS in the left ear.  f) At the facility, where the employer did not record a STS on the 2015 OSHA 300 log.  A STS was not recorded on January 14, 2015, where an employee had a STS in the right ear.

29 CFR 1904.32(a)(1):  At the end of each calendar year the employer did not review its OSHA 300 log to verify that the entries were complete, accurate, and correct any deficiencies identified:  a) At the facility, where the 2014 OSHA 300 log was not complete and accurate.  An illness that occurred on August 15, 2014, where an employee passed out at the worksite and was taken to the emergency room was not recorded.  b) At the facility, where the 2014 OSHA 300 log was not complete and accurate.  An injury that occurred on April 12, 2014, where an employee injured his shoulder while stopping his fall at the worksite and was treated for the same injury for approximately two months was not recorded.  c) At the facility, where the 2014 OSHA 300 log was not complete and accurate.  An injury that occurred on February 19, 2014, where an employee injured his back while riding a bike at the worksite was not recorded.  d) At the facility, where the 2014 OSHA 300 log was not complete and accurate.  An injury that occurred on September 7, 2014, where an employee fell from a platform and injured his right elbow was not recorded.  e) At the facility, where the 2014 OSHA 300 log was not complete and accurate.  An injury that occurred on March 15, 2014, where an employee bruised his knee climbing a scaffold ladder was not recorded.
