29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Dunkin' Donuts construction site, 2545 W. 12th St. in Erie, PA.  On or about December 16, 2014, employees performing decking on the roof of the building were exposed to a 14-foot fall from the unprotected edge where hoisting materials could be received.  The crane was not in operation and no materials were being hoisted but employees working near the edge were not protected from falling to the lower level. In addition, personnel were not protected from falling while erecting the parapet.

29 CFR 1926.501(b)(2)(i): Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:     a) Dunkin' Donuts construction site, 2545 W. 12th St. in Erie, PA.  On or about December 16, 2014, employees constructing the decking of the roof were exposed to a 14-foot fall while performing the leading edge work.  A fall protection system was not utilized during the installation of the roof decking.
