29 CFR 1910.29(a)(2)(v): All exposed surfaces shall be free from sharp edges, burrs or other safety hazards.  Employer did not provide manually propelled mobile ladder with exposed surfaces that were free from sharp edges, burrs, or other safety hazards.    a/ In the receiving area a 6-step mobile ladder stand with visible damage was in use by employees. The ladder platform was separated from the back legs and the guardrail. The bottom step was separated from the stile and the back left (facing front of ladder) leg was attached to the base and caster by unidentifiable wire.  b/ In the layaway department a 6-step mobile ladder stand with visible damage was in use by employees.  The front left(facing front of ladder) stile or side rail was detached from the base of the ladder.   The front right (facing front of ladder) stile or side rail was missing from the base of the ladder.

29 CFR 1904.32(b)(3): How do I certify the annual summary? A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete.  Employer did not provide OSHA 300 Logs that had been certified by a company executive showing that he or she reasonably believes the annual summary is correct and complete.  The OSHA 300 Log of Work-Related Injuries and Illnesses and OSHA 300A Summary of Work-Related Injuries and Illnesses were provided for the following:  a/ Year 2012 b/ Year 2013 c/ Year to Date 2014
