The working levels of the scaffolding were not fully planked or decked, exposing the employees to a fall hazard of greater than 19 feet to the ground below.

The employees were not protected from falling by guardrails and end stops and as such were exposed to fall hazards of greater than 19 feet to greater than 25 feet. b) Madison Ave., & 10th St., Lakewood, NJ: On or about 12/17/2015, two employees were working from a pump jack scaffold installing EFIS. The employees were not protected from falling by midrails and end stops and as such were exposed to a fall hazard of greater than 24 feet.
