29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. a. On the underside of the bridge where employees used an elevated aerial lift to gain access to the false deck, employees climbed the lift's guardrails to gain access to the false deck, thereby being exposed to a 20 foot fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. In the elevated basket of the aerial lift where employees were working on and accessing the underside of the bridge, a personal fall arrest system or restraint system was not used, thereby exposing employees to a 20 foot fall hazard.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. At the edge of the false decking on the underside of the bridge where employees entered and exited the decking, fall protection was not used, thereby exposing employees to a 20 foot fall hazard.
