29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    The employer does not ensure that employees working in areas where there is a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were protected by protective helmets.  This violation was observed on or about December 08, 2014 when employees were exposed to a head injury from impact when walking beneath a scaffold that was being disassembled without wearing protective helmets.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure employees working in areas where there is a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, are protected by protective helmets.

29 CFR 1926.451(h)(2)(i): The area below the scaffold was not barricaded, and employees were not prohibited from entering the hazard area.    The employer does not ensure that the area below the scaffold was barricaded, and employees were prohibited from entering the hazard area. This violation was observed on or about December 08, 2014 when employees were exposed to struck-by hazards when exiting the building whose exit discharge was below a scaffold being dismantled.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the area below the scaffold is barricaded, and employees are prohibited from entering the hazard area.
