29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about 01-09-15, an employee performing residential construction was walking/working on top of trusses at approximately eighteen (18) feet above lower surface without utilizing fall protection.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  a)  On or about 01-09-15, an employee was working on a roof utilizing an ARCO Building Systems guardrail on an eave edge nine and one half (9.5) ft. above the lower surface without the top surface rated to withstand 200 lbs. of force without failure.  The posts were not spaced at a maximum of eight (8) ft. apart, the rails were not secured, and an open-edge guardrail holder was used on the eave edge.

29 CFR 1926.502(b)(5): Midrails, screens, mesh, intermediate vertical members, solid panels, and equivalent structural members were not capable of withstanding, without failure, a force of at least 150 pounds (666 N) applied in any downward or outward direction at any point along the midrail or other member.  a)  On or about 01-09-15, an employee was working on a roof utilizing an ARCO Building Systems guardrail on an eave edge nine and one half (9.5) ft. above the lower surface without the midrails rated to withstand 150 lbs. of force without failure.  The posts were not spaced at a maximum of eight (8) ft. apart, the rails were not secured, and an open-edge guardrail holder was used on the eave edge.
